Instructions

1. Please complete a feedback form embedded below. You must click the "submit" button to submit your form. If you are on a mobile device and have difficulty seeing the form, please try a computer. Make sure you enter the same email that you use to log in to this course.

2. You will receive a confirmation email within 5-30 minutes. If you do not receive the email in your inbox, please check your spam folder or promotions tab in gmail. If it is not there, it means the form did not go through or there was a typo in your submission. Please try clearing the cache in your browser or using a different browser/device and submitting the form again.

3. You will be sent a PDF certificate via email within 48 hours.
